Nigeria Travel Documents

Click on the files below to view, fill in, and print the travel documents required for travel to Jos, Nigeria with Back2Back Ministries.  Please read the directions carefully and have the appropriate documents notarized.

Here is a checklist so you know what forms are needed and where they should be sent.

Passport List – to be emailed to Chris Ramos at
                              least 2 months prior to trip
  ¨  Name of each participant as it appears on
            their passport
  ¨  Address
  ¨  Passport number
  ¨  Birthday
  ¨  Vocation 

  

Adult Forms – RETURN TO TRIP COORDINATOR
  ¨  Emergency Information Form (notarize) (2 COPIES)
      o   Trip Coordinator keeps original and sends copy to B2B
      o   There are two pages of this form
  ¨  Liability Release Form (notarize)
  ¨  Agreement Policy - Nigeria
  ¨  Skills Assessment - Nigeria

 

Minor Forms (applicable to children under 18 years old) –
                          RETURN TO TRIP COORDINATOR
  ¨  Emergency Information Form (notarize) (2 COPIES)
      o   Trip Coordinator keeps original and sends copy to B2B
      o   There are two pages of this form
  ¨  Liability Release for Minors Form (notarize)
  ¨  Permission for Minors to Travel - Nigeria (notarize)
      o   Both parents must sign the top portion and have notarized
      o   If only one parent is traveling with the minor, the bottom portion must be filled out
                 and notarized
      o   If both parents are traveling with the minor, this form does not need to be filled out
      o   Trip Coordinator keeps this form
  ¨  Agreement Policy - Nigeria
  ¨  Skills Assessment - Nigeria

 

All Participants – to take while traveling
  ¨  Valid passport with Nigerian visa (must have more than 6 months validation
            remaining)
  ¨  Vaccination card with the Yellow Fever Vaccination seal
      o   You’ll receive this card once you are vaccinated
